Employees Retiring Now

Employees who are retiring and are benefit eligible are required to complete the following 3 forms and submit them to the Human Resources Department as early as possible (but no more than 30 days) prior to their retirement date.

  1. DMS Billing Form for Retirees with Altus Dental
  1. TASC Billing Enrollment Form - for Retirees with Davis Vision
