Due to Massachusetts licensing requirements, you must submit for each child enrolled in our LICENSED camps, a
Health Information Form (PDF) and a
Certificate of Immunization (PDF) (current within last 18 months and signed by a physician) with proof of chicken pox and polio immunity.
In-person registrations WILL NOT be processed without the child's health records at the time of registration. Online registrations,
must submit forms within one week of registration. Please note that these forms must be provided each year.
