Our licensed summer camps require current physical and immunization records in order to be licensed by the Board of Health. The oversight of this requirement falls on a single staff in the office, making it difficult to keep track of non-compliant campers.
You must email a recent physical and immunization form for each child attending a licensed camp. Physical and immunization records must be within 18 months from the start of camp. A doctor's standard form used for schools and camps works perfectly for this purpose since it has the physical information and the immunization records on it. You may email a PDF of the forms to email@example.com. If you need help with this, please email the office so we can assist you.
Beginning in Spring 2024, current physical and immunization records will be required prior to any online licensed camp enrollments.
1) Login to your ActiveNet online account here
2) Scroll to the bottom of your list of family members and select “View Family Members Schedule”
3) Select Family Members whose schedule you’d like to view, and toggle the calendar between Week/Month as you choose
